Un hôpital à la dérive
La crise de la santé en Colombie. L'hôpital de Saint-François d'Assise est le plus important hôpital public du département du Chocó en Colombie. 500 000 personnes en dépendent. Mais comme de nombreux hôpitaux colombiens, il est au bord de la faillite.
Temps d'attente prolongé en dépit de pronostic vital énoncé, limite des capacités d'intervention, manque de médicaments ou encore défaillance du matériel sont quelques unes des complications auxquelles les malades doivent faire face.
Les bâtiments sont dans un état avancé de délabrement. Les infirmières quant à elles gèrent au quotidien cette situation bancale. Elles ne sont pas toujours payées, certaines accumulant 7 mois de retard de salaire. Et pourtant elles restent, seuls anges gardiens des patients dans cet hôpital à la dérive.
Plus généralement, la série porte un regard sur la crise de la santé en Colombie. La santé est un enjeu global, et son financement un casse-tête pour la plupart des États. Au cours des 20 dernières années, les scandales de détournement d'argent et les faillites d'hôpitaux publics se sont succédées dans ce pays.
Pourtant, la réforme entreprise au cours des années 1990 devait conduire à une restructuration efficace et économique de l'accès aux soins avec la création des EPS. Ces entreprises gèrent l'affiliation et les budgets de la santé. Mais pour réaliser des économies, les EPS ont alourdi les démarches que les malades doivent effectuer dans les administrations pour justifier les soins dont ils ont besoin.
En conséquence, beaucoup perdent l'espoir de recevoir un traitement. Ils abandonnent toute tentative, voire meurent dans l'attente d'obtenir une autorisation.
C'est avec beaucoup de cette ironie propre au désespoir que les Colombiens ont trouvé une expression pour résumer leur parcours du combattant dans les dédales de l'administration. Ils appellent cela el paseo de la muerte, la promenade de la mort.

A Hospital Adrift
The healthcare crisis in Colombia. The San Francis of Assisi hospital is the most important public hospital of the department of Chocó on which depends 500 000 people. But like numerous Colombian hospitals, it is on the verge of bankruptcy.
Long waiting times despite life-threatening conditions, limited response capabilities, lack of medicine or failures equipment, these are some of the complications that the patients are facing. The buildings are in a deteriorated state.
The nurses manage day-to-day this shaky situation. They don't always get paid, some of them are waiting for seven months in delaying salary payment. However the nurses stay, as the only angel-guards for the patients on a hospital adrift.
Overall, this series looks back at the Colombian health system crisis. Health is a global issue, and its funding scheme is a headache for most States. The scandals of misappropriation of funds and the healthcare institution banckruptcies have followed one another since 20 years. The quality of care continues to degrade. The process reform of the 1990s should lead to an efficient and economical reorganization of the access to healthcare, with the creation of the EPS.
These bureaucratic entities, similar to the HMO of US, have to affiliate population to the healthcare system and manage the healthcare budgets. But to realize some savings, EPS made the procedures more complicated. The patients have to justify in an administrative way the care they need. Lots of requests are denied and many of the sick people lose hope to receive medical treatment. They give up any attempts, and some of them die expecting to obtain a clearance.
With this paintful irony that only allows the hopelessness, Colombians have a saying to describe their obstacle course in the administrative mazes. They call it "el paseo de la muerte", the death's path.
